If turning a door handle, wringing out a cloth, or twisting a jar lid brings on a sharp or aching pain in your wrist, you may be dealing with wrist tendonitis.

What is it?

Tendonitis means inflammation of a tendon, the tough cord that connects muscle to bone. In the wrist, several tendons run from the forearm muscles, across the joint, and into the hand, allowing you to bend, straighten, and twist your wrist and fingers. When one of these tendons becomes irritated, usually from repeated movement, it can become swollen and tender, and twisting or gripping motions become uncomfortable or painful.

Why does it happen?

Wrist tendonitis is most often caused by repetitive activity, such as typing, using tools, playing racquet sports, or manual work that involves frequent twisting or gripping. It can also follow a sudden increase in an activity your wrist isn't used to, or a minor strain that hasn't had time to settle. Poor wrist positioning during repeated tasks, and general wear and tear with age, can also play a part. It is a common problem, affecting a significant proportion of adults at some point, particularly those in physically repetitive jobs or hobbies.

When should I see a hand surgeon?

Most cases of wrist tendonitis settle with simple measures, but it is worth seeking a specialist opinion if the pain persists for more than a few weeks despite rest, if it is severe or worsening, or if you notice significant swelling, weakness, or difficulty using your hand for everyday tasks. You should also seek prompt assessment if the wrist becomes hot, red, or you feel generally unwell, as this can occasionally suggest a different underlying cause that needs urgent attention.

How is it treated?

Initial treatment usually focuses on rest, activity modification, and reducing inflammation, sometimes with the help of a splint, ice, and anti-inflammatory medication. Hand therapy can be valuable for guiding a gradual return to activity and correcting movement patterns that may have contributed to the problem. When symptoms persist, a targeted steroid injection may be considered. Surgery is rarely needed for wrist tendonitis and is generally reserved for the small number of cases that do not settle with these measures.
